About the role
Herts Health GP Federation is delighted to offer a position for a Clinical Pharmacist who wants to make a difference and the opportunity to support our two networks of GP Practices in Hertsmere. We see the role of the Pharmacist as critical to realising our ambitions to improve outcomes. As one of the first joint posts across our two PCNs, the Pharmacist will be in a unique position to develop new care protocols, identify priorities and shape our longer-term strategy. The practice populations of the Practices are diverse and this provides an interesting variety of work for anyone hoping to join our Network.
We would expect the pharmacist to work across the Networks to achieve joint aims for all eight practices, as well as looking at individual work within the practices. This would then be built on in future years. The role is diverse and will likely evolve depending on the needs of the PCN and the ability of the post holder. You can expect the role to include risk stratification, patient facing medicines support, audit work to assist in meeting practice targets, repeat prescribing (where applicable), unplanned hospital admission, medicine safety, CQC preparation, implementation of local/national guidelines and general quality improvement across our region.
We are recruiting for 5 Clinical Pharmacist roles; including one Senior Clinical Pharmacist who will take on responsibility of team manager.
